HOW TO GET STARTED

  • Reach out to Hands in Motion Homecare LLC by phone, email, or through our website. We will complete a client inquiry form with you unless you have already filled it out on the website. If needed, we will follow up to make sure we have all required information and ask any additional questions.

  • We will review your eligibility to make sure you qualify for services and determine the best options available based on your needs.

    • If you have a Medicaid Waiver, we will work together to contact your waiver case manager to coordinate services, confirm authorizations, and ensure everything is in place to begin care.

    • If you are a private pay client, we will complete a consultation to review your care needs, discuss available services, and go over pricing and scheduling options.

    • If you have long-term care insurance, we will assist in verifying your benefits and help guide you through the process to access and use your coverage for services.

  • We will meet in person to complete the intake process and develop a personalized service plan based on the client’s needs, safety, independence, and preferred method of communication.

  • We will work on matching the client with a caregiver and setting up a schedule that works best for the client and family.

  • Once everything is confirmed, services will begin. We will continue to communicate, monitor care, and make updates as needed.
